Primary Purpose:



The Youth Case Manager will provide trauma informed, youth centered, culturally and linguistically appropriate, community-based services to immigrant youth that reside in the local community. The Youth Case Manager is responsible to promote the safety and welfare of the children receiving case management by conducting safety screenings and needs assessments, and by monitoring service plans that support youth to reach their basic needs and adjustment goals. In addition, the Youth Case Manager will provide Know Your Rights Training and conduct outreach and education to community service providers to supplement existing services that do not adequately address the unique needs of children.



Responsibilities:


  • Conduct in- person home visits with children.
  • Conduct Safety Screenings
  • Conduct child centered needs assessments.
  • Develop individualized strength-based goal-oriented service plans.
  • Coordinate referrals in relevant service need areas such as legal, education, medical, mental health, etc.
  • Support children and responsible adult with enrollment and access of services in community
  • Provide psychoeducation to children and responsible adult as needed.
  • Support children and responsible adult to cope with family reunification stressors.
  • Monitor progress of goal completion.
  • Conduct Know Your Rights trainings.
  • Monitor the safety and well-being of children including but not limited to indications of trafficking and abuse and neglect.
  • Serve as a mandated reporter and work with child welfare as needed.
  • Network and develop partnerships within the local community to educate providers on children’s needs and to reinforce/increase partnerships in relevant areas.
  • Write case notes; enter data; complete reports in a time sensitive manner.
  • Support team members to troubleshoot crisis issues that arise with children and responsible adult.
  • Participate in local and regional meetings that pertain to service provision for the target population.
  • Develop referral relationships to address service needs.
  • Perform other related duties and special projects as requested.


Qualifications:

  • Minimum education of aMastersin Social Work (MSW) or an equivalent degree in education, psychology, sociology, or other behavioral science.
  • Licensed or license eligible preferred.


Experience:



Minimum of 3 years’ experience working with children and youth, family reunification/ preservation, and or foster care/child welfare; previous work experience and interest working with foreign born populations is required. Ability to provide trauma-informed psychosocial education and case management is required.

Other Skills:



Proficient in Microsoft Officeapplications.Bilingual Spanish/English fluency required (spoken and written). Superior written and verbal communication skills. Strong organizational and time management skills. Must be able to travel frequently and independently throughout relevant region. Flexibility to work evenings and weekends as assigned.Must be able to pass an FBI background check and a child abuse/neglect check.Must have a valid driver’s license, reliable transportation, insurance and adhere to the CWS Driving Policy.

Other Skills:

Computer literacy, using word processing and database applications, excel and PowerPoint programs. Spanishfluency required.

Special Requirements:


  • Covid vaccine required
  • Must complete motor vehicle record check.
  • Must be willing and able to travel.
  • Background check required.
