- Location: Holy Cross Health has two hospitals and four healthcare centers all a short driving distance from Washington DC and Baltimore, MD
- Reporting to the Manager of Social Work
- Social Worker will provide assessment, counseling, referral and discharge planning services to all age groups of patients treated at Holy Cross; provides supportive services to patients and their families.
- Social Workers are focused on discharge and initial inpatient admission and appropriate community-based services.
- Reviewed clinical assessments and determining appropriate levels of care, determining if additional clinical services are needed for infants to elderly patient population.
- Effectively handled member crisis calls by identifying risk, ensuring safety, and coordinating timely access to care.
- State of Maryland LSWA, LGSW, LCSW or LCSW-C licensure (or license eligible).
- Masters Degree in Social Work.
- One year of social work experience in an acute-care setting preferred. Demonstrated ability to work with all age groups of patients, with an emphasis on adult and geriatric patients. Able to communicate effectively.
