Job Summary:

The Day Habilitation Service Manager (DHSM) ensures documentation and delivery of services in compliance with Medicaid regulations, CARF, and Bay Cove standards. The DHSM performs case management duties and supervises program staff working with assigned program participants. Areas overseen include developing, implementing, and supervising daily program activities. Also Day Habilitation Service Plans (DHSP) and Individual Support Plan (ISP) execution, maintaining client records, processing referrals for new program applicants, scheduling and coordinating ISP meetings and documentation. DHSM's are involved with maintaining relations with those receiving program services, their supporters and other helpers, and representatives of Mass. Health and the Department of Developmental Services. This is an exempt position.

Supervisory Responsibilities:

  • Supervision of daily operations of program activities and assigned direct care staff to ensure compliance to program schedules, Day Habilitation Service Plans, and standards of care.
Essential Functions of Position:
  • Serve as Day Habilitation Service Manager (Medicaid regulatory title). Supervises and trains assigned program staff in areas such as therapeutic supports, curriculum, activities, data collection, and implementation of DHSP goals and objectives. Schedules staff for out of program training as needed to meet training needs.
  • Performs assessments of developmental and functional skills, and facilitates meetings with program participants to develop DHSP goals and objectives, including those that promote the development of self-determination and decision-making skills. Reviews members DHSP's and requests updates as needed. Conducts and documents quarterly reviews of DHSP's with interdisciplinary team and makes updates as needed.
  • Develops tools, trains staff, and oversees the recording of data to measure progress towards objectives; ensures that data and subsequent reporting is in compliance with funding source and agency requirements as well as program mission and standards.
  • Develops and implement daily activity schedules for the program that provide teaching and skills training in accordance with objectives and goals set forth in individual's DHSP and ensure compliance with Day Hab regulations. Performs direct care duties as needed to ensure staffing ratios and physical needs for program participants are met.
  • Manages referral and admissions for assigned groups within program, including completion of pre-admission documentation, setting up records, conduct and submission of Severity profile.
  • Provides leadership as a member of the Day Hab management team. Conducts service plan meetings, leads team meetings, and documents meeting minutes and supervision.
  • Performs operations duties as assigned, including ordering supplies; quality management reviews of records; preparation of billing from attendance records.
  • Maintains good working relationships with collaborating programs both within and outside of the Bay Cove Human Services network, as well as with representatives of funding sources. This includes representing Bay Cove and the program at official meetings with funders and other providers.
  • Carries out other duties as assigned by the Director of Day Habilitation
Preferred Requirements for the position:
  • Previous experience consisting of service management in a health care setting. Demonstrated knowledge of service deliveryregulations and ability to develop functional tools for documentation, implementing plans, and participating in IDT and related meetings.
  • Demonstrated knowledge and understanding of Intellectual Disabilities and other Developmental Disabilities and principles to promote skills development.
  • Demonstrated excellent written and verbal communication skills.
  • Demonstrated management, organization, and documentation skills. Knowledge of computer and word processing (eg, use of Word and Excel) to create forms and documents.
  • Current MA driver's license.
  • An acceptable CORI (criminal record) assessment.
  • Must have had a TB screening within the past 12 months, & required every year.
  • A COVID-19 vaccination is a requirement of the position. One COVID-19 shot is acceptable, contingent on the individual receiving the second shot within the allotted time frame.
