CORE JOB SUMMARY

The University of Miami is currently seeking a Patient Navigator to work at Sylvester Cancer Center (SCCC) in Miami. The incumbent facilitates patients' ability to obtain services by identifying barriers to care, coordinating the development of treatment plans, and monitoring the implementation and follow-up of services ordered by caregivers.

CORE JOB FUNCTIONS

1. Conducts patient evaluations to obtain information necessary for the development of an effective treatment plan.
2. Ensures patients have access to insurance and social service programs for which they are eligible.
3. Provides patient reminders of upcoming appointments and acts as liaison between patients and providers.
4. Identifies and resolves real or perceived barriers to care.
5. Educates and counsels patients on issues related to their health and life situations.
6. Maintains current knowledge of community health, welfare resources, and any other programs available to patients.
7. Adheres to University and unit-level policies and procedures and safeguards University assets.

Department Specific Functions

Primary Duties and Responsibilities (For Non-exempt Employees Include Percent of Effort):

  • Assists the Director of Social Services with specific duties for program development and gathering of metrics.
  • Assists the social work team in setting up appointments for patients that need help making appointments for various services.
  • Assists the social work team in faxing referrals to appropriate agencies and making follow up calls to the patients/caregivers and agencies/organizations to ensure that services are rendered.
  • Assists the social work team in providing services and referrals to the community to meet the needs of cancer patient, caregivers, and survivors.
  • Assists the social work team in preparation and advertisement of the oncology specific support groups or other educational programing.
  • Assists the social work team in various ways to advocate for the patient.
  • Assists the social work team in the completion of various documents.
  • Assists the social work team in the administration of a specialized program.
  • Assists the social work team in paperwork associated with discharge plans for the patient.
  • Supports and maintains the standards of the Sylvester Comprehensive Cancer Center, Psychosocial Oncology Department, and the social work profession.
  • Enters information daily in database.
  • Performs other duties as assigned.
  • Assists with other clerical duties, not limited to faxing, scanning, copying

CORE QUALIFICATIONS

Education:

High School diploma or equivalent required

Experience:

Minimum 3 years of relevant work experience required
