Department Information

For more than 90 years, the Department of Pediatrics at Indiana University School of Medicine has provided comprehensive medical training for medical students, residents and fellows in addition to lifelong learning opportunities for practicing physicians. The department consists of 19 specialty divisions based at Riley Hospital for Children at Indiana University Health, which is ranked as one of America's Best Children's Hospitals in 10 out of 10 specialties in US News and World Report.

The Indiana Complex Care Coordination Collaborative (IC4) embeds and coaches nurses in primary care pediatric and adult practices to provide care coordination for children with medical complexity and adults with intellectual disability, as an Indiana Medicaid-funded project with close collaboration with Indiana Title V. The overall goal is to better organize care to improve the healthcare and quality of life for eligible individuals, as well as their families' well-being and the cost-effectiveness of their care within a sustainable model.

Job Summary

Department-Specific Responsibilities

  • Responsible for launching and maintaining the Be Strong Families Parent Café model for the families of the IC4 program.
  • Works closely with families, Indiana Family to Family (F2F) and community partners to coordinate virtual and in-person Parent Cafés/Vitality Cafés across the state of Indiana focusing on parents of children with disabilities and supporting the connection to other Parent Cafés being performed in the state.
  • Works with families to identify café topics, identify family leaders who would like to supporting running the cafés, and is the social work support during cafés. This person will be responsible for preparing and managing café data to report to the University Team.
  • Works within the university to provide social work guidance to Nurse Care Coordinators as needed.
  • Works with the Program Director to build relationships with outside social work support and community resources to create a network for those practices who do not have direct access to social workers.
General Responsibilities
  • Provides medical social work services to inpatients and outpatients and their families including psychosocial assessment, supportive counseling, crisis intervention services, trauma focused care, discharge planning, information and referral, and participation in multidisciplinary treatment team, preadmission planning when appropriate; discharge planning; coordination of community resources.
  • Participates in consultative multidisciplinary treatment team, coordination of community resources, maintain database of patient population, treatment, intervention and outcomes.
  • Coordinates follow-up care and consultation with allied health professionals, the Department of Child Services, and local law enforcement.
  • Documents patient activity as required by department.
  • Serves as liaison to community agencies regarding patient problems or issues related to medical diagnosis.
  • Identifies and pursues opportunities to enhance knowledge base related to programs, articles, staff education, or community projects.
  • Complies with all policies and standards, accurately documenting the medical record and serving as a resource when needed.
  • Complies with required standards for medical/legal reporting situations, and/or educates other professionals with responsibility for reporting.

Qualifications

EDUCATION

Required
  • Master's degree in Social Work (MSW) or similar degree approved by the Council on Social Work Education
WORK EXPERIENCE

Required
  • 2 years experience dealing with specific issues of child maltreatment
Combinations of related education and experience may be considered

LICENSES AND CERTIFICATES

Required
  • Licensed Social Worker LSW in the state of Indiana upon date of hire
Preferred
  • Licensed Clinical Social Worker LSCW in the state of Indiana upon date of hire

Working Conditions / Demands

This role requires the ability to effectively communicate and to operate a computer and other standard office productivity equipment. The position involves sedentary work as well as periods of time moving around an office environment and the campus. The person in this role must be able to perform the essential functions with or without an accommodation.

Work Location

Indianapolis, Indiana

This position is eligible to work a hybrid schedule (mix between remote and in-person work), subject to change in the future based on university policy and business needs.

Advertised Salary

The salary range for this position is $58,000 - $68,000. Salary determination is based on experience, skill level, education, and training.
