Description:

Responsible for all Central Intake functions including establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, confirming appropriateness for admission into Loyola's Home Infusion services, and coordination of needed

services. The individual conveys a professional and positive image of Loyola Medicine during

interactions with patients, familes, physicians, staff members, and internal departments. The individual provides service reflective of the mission, vision, and values of Loyola Medicine.

Position Requirements:

Minimum Education:

Required: Associates Degree

Preferred: Bachelors Degree

Specify Degree(s): Nursing

Residency: N/A

Minimum Experience:

Required: 3-5 years of previous job-related experience

Preferred: 6-10 years of previous job-related experience

Managerial Experience: N/A

Licensure/Certifications:

Required:

CPR (Cardiopulmonary Resuscitation)

Current Registered Nurse License State of Illinois

Valid Illinois State Drivers License

Other: Homecare or Hospice Nursing certification is preferred. Proof of car insurance is required
