Field Nurse Practitioner: Hospital at Home Care

*Company Vehicle provided for field visits.

Location: Field Based - This position requires driving and in-person visits to patients' homes in Clark County, Nevada.

Work Shift: 5 days a week, Monday thru Friday, 8 am to 5 pm

Must have current, unrestricted RN and NP licenses in the state of Nevada.

The Field Nurse Practitioner is responsible for providing in-person and virtual healthcare services to patients at home, in collaboration with the attending physician, through advanced assessment and identification of health states including abnormal conditions, diagnosis, treatment and evaluation. This team member will deliver a mix of home-based palliative care, post-acute, chronic disease and primary care management for homebound patients, and higher acuity "Hospital at Home" care to complex patients. Responsible for collaborating with the patient's other physicians and providers, and their family members to develop complex plans of care in accordance with the patient's health status and overall goals and values.

How will you make an impact:

• Utilize evidence-based clinical practice standards to provide direct clinical services for patients within prescribed guidelines, within the scope of practice, and under supervision of a collaborating physician
  
• Acquire patient health histories, performing physical examinations, ordering and interpreting complex diagnostics, and making critical decisions regarding a wide variety of health situations, including high risk and crisis situations
  
• Perform health assessments, specialized risk assessments, and developing plans of care for patients facing acute and chronic and health conditions.
  
• Make routine in-person (and virtual) visits to patient homes as clinically necessary and in accordance with Medicare guidelines
  
• Document encounters precisely and timely on the appropriate EHR system, and clearly communicating plans of care and/or any changes of condition to patients and team members.
  
• Participate in Rotating On-Call program which may include after-hours, weekend and holidays
  

Minimum Requirements:

• Requires an MS in Nursing and minimum of 1 year of related experience with clinical emphasis for nurse practitioner ; or any combination of education and experience, which would provide an equivalent background.
  
• Current, unrestricted RN license and NP license in the state of Nevada is required.
  
• Possession of DEA registration or eligibility..
  
• For Carelon Health business units, satisfactory completion of a Tuberculosis test is a requirement for this position.
  

Preferred Skills, Capabilities and Experiences:

• Experience in complex care, wound care, home health experience is a plus.
  
• 2-5 years of experience working in an acute care setting such as ICU, CCU, telemetry, emergency medicine is a plus.
  
• Proficient in use of EMR and other technology use.
  
• Bilingual communication skills.
  

For candidates working in person or remotely in the below location, the salary* range for this specific position is $110,880 to $166,320
