The Homestead at Rochester Rehab & Living is seeking a Director of Social Services to join our awesome team!

The Director of Social Services will organize and direct social services functions through direct service with the resident and family throughout pre-admission and admission processes, resident adjustment, ongoing and discharge services.

Essentials:

  • Leads and directs Social Service Department
  • Coordinates and leads admission transfer and discharge team
  • Completes assessments of residents as necessary for care planning, MDS, RAPS, PPS and other required documents
  • Initiate and/or participate in multidisciplinary care conferences held for residents, as appropriate to state and federal guidelines
  • Initiates reporting and documentation in the medical record
  • Serve as facility Discharge Planning Coordinator
  • Serve as an advocate for residents, upon request of the residents, by attendance and participation in Resident Council meetings; promote and adhere to Residents Rights Act
  • Advise and recommend solutions to the Executive Director and Director of Nursing of major problems or concerns that residents or their family members are experiencing
  • Advise and assist Nursing and Recreational Departments in ensuring residents have the necessary and adequate clothing needs, health aids, etc.
  • Maintain supply of admission packets, application materials and necessary department forms
  • Maintain monthly Social Service referral/inquiry report
  • Develop and promote positive public relations and community resources through contacts with social service personnel in area hospitals, welfare departments and the Social Security Administration
  • Administer personnel functions of Social Services Department, when appropriate
  • Participate in department budgeting and planning processes
  • Serves on committees as required by Executive Director including, but not limited to: CQI, Safety, Management Admissions, Marketing
  • Attends resident and family council meetings
  • Serves as rights advocate liaison to the facility
  • Advises and recommends solutions to the Executive Director and Director of Nursing of major problems or concerns that residents or their family members are experiencing
  • Ensures residents have necessary and adequate clothing, health aids, etc.
  • Develops statistics for Executive Director on operation of department including admission/discharge/referral/inquiry statistics
  • Acts as the facility Resident Protection Coordinator, coordinating investigations of incidents of potential abuse/neglect at the direction of Executive Director.
  • Works directly with DON, HR and department managers to complete thorough investigation following and utilizing VOANS policies and forms
  • Serves as the Grievance Official
  • Attends required in-services and completes assigned online modules
  • Performs other duties or special assignments as directed by the Executive Director
Required Qualifications:
  • Bachelor's Degree in Social Work or related degree in social sciences; licensed where applicable by the state; state requirements may dictate supervision and degree type.
  • Minimum of one year of social work experience, preferably in a long-term healthcare setting; knowledge of long-term healthcare and public health settings; familiarity with Federal and State regulations and requirements for long-term care.
