Purpose

The CRS Case Manager provides virtual and in-person assistance to eligible refugee and migrant populations, including Cuban-Haitian entrants, Ukrainian humanitarian parolees, Afghan SIVs, and other qualified newcomers. The Case Manager will complete client intake and assessments to determine program eligibility, developing a service plan and connecting eligible clients to appropriate benefits and resources. The Case Manager provides trauma-informed and client-centered services to support individual client needs, offering a range of educational, financial, medical, social, vocational, and other services and resources based on client need and eligibility. The Case Manager will be based out of the CWS Fort Collins office, with travel to local government and community service agencies and the ability to provide services remotely to clients throughout the State of Colorado. The Case Manager will work directly with incoming families and will be required to complete a background check, confidentiality agreement, and training modules, treating all individuals with compassion, understanding, and respect. Spanish language ability is highly preferred.

Responsibilities

  • Provide case management to a caseload of immigrants and refugees; Ensure the best possible service experience for each client, providing appropriate support, resources, and referrals to connect newcomers with benefits and resources.
  • Conduct intake interviews and assessments for Cuban-Haitian entrants, Ukrainian parolees, Afghan SIVs, and other refugee and immigrant newcomers to determine program eligibility; Create individualized service plans to meet the needs of individual clients and families.
  • Provide appropriate and timely referrals to ensure clients are connected to human assistance programs, educational resources, housing support, and other programs to assist with long-term self-sufficiency; Assist clients to access health and mental health resources, employment and job support, and immigration legal services by assessing and evaluating each client's distinct needs.
  • Support clients with RCA cash assistance distribution and tracking; Provide donations and other forms of assistance, as needed.
  • Regularly connect with local services providers, community-based organizations, and other agencies throughout the State of Colorado to ensure that clients are provided the basic tools necessary to meet their established short-term goals and objectives.
  • Work regularly with co-sponsors, faith-based, and community groups to support the successful resettlement of clients. Provide training, guidance, and resources to co-sponsors assisting clients statewide, in a virtual capacity.
  • Assess and monitor client progress at regular intervals to ensure progress on goals is made, designated resources are maximized and modify initial resettlement plans and other case file documents as required.
  • Ensure compliance with case file management and reporting requirements, including timely data entry in hard-copy files and databases; Manage and use DocuSign and other online platforms to ensure confidential data transfers.
  • Travel regularly throughout the service delivery area (home visits, agency visits, service provider visits etc.).
  • Participate in all program meetings, staff development activities and fully engages as a member of the team.
  • Other related duties as assigned.

Qualifications

Education and Experience:
  • Bachelor's degree or equivalent professional experience.
  • Experience working with refugees or immigrants, and previous case management experience preferred. Experience with data tracking and case file documentation preferred.
Other Skills:
  • Strongly preferred that candidates are bilingual in English/Spanish or English/Ukrainian.
  • The successful candidate must be computer literate, with proficiency in MS Office Suite (Word and Excel), Docusign, and internet applications.
  • Ability to utilize communication platforms and provide virtual case management required.
  • Exceptional organizational and time management skills are required.
  • Excellent written and verbal communication skills are essential.
  • Ability to work in a multi-cultural environment, with compassion and respect for diverse populations required.
